I would change his diet for a few days, see if that helps. Cut out anything spicy or citrus. That doesn't look like regular diaper rash.

Is he teething? Mine always had horrible rashes when they were teething or maybe you could just start potty training? he he! :)
Is it maybe a yeast infection instead of a regular diaper rash? might need a prescription? Hope he gets better soon. Owie!(you probably have no idea who I am, I am a babynames friend of Jessie's. Not a weirdo! Promise! ;-)
I hope it has gotten better by now, but if not Try Butt Paste (get it at Target) That worked really good on my niece.
